Group Package: 10 lessons, 5 weeks. 2 - 1 hours sessions each week;
           $250/each attendee (min of 4 players - max of 6)   

Individual Lessons: $65 per hour  (Quantity discounts available for multiple individual lessons).
All players must be accompanied by a parent or guardian. Your registration will not be complete until
you and your parent or guardian sign the release form (if not currently a USAV member) and pay your  
fees.
Upon receipt of this form Coach Kind will contact you to schedule a specific time and make payment
arrangements.
